$1,000 ACCIDENT MEDICAL EXPENSE BENEFIT
This benefit pays up to $1,000 per accident in covered expenses within 90 days of a covered loss, if you are charged by a hospital or doctor, or certain other charges, up to the maximum listed, if you are injured in a covered accident. Subject to a $100 deductible per covered accident. Accident Medical Expenses are only payable in excess of other insurance coverage(s). This benefit covers members, under age 65, and their eligible dependents.

$5,000 ACCIDENTAL DEATH & DISMEMBERMENT BENEFIT
Pays the beneficiary up to $5,000 for the member’s death or loss of certain body parts in a covered accident or a portion of that amount for the accidental death of a family member (50% for covered spouse, 60% if there are no dependent children; 20% for covered dependent children, 25% if there is no covered spouse). This benefit covers members, under age 65, and their eligible dependents.
